How are you recording voltage-drop tests so another tech can repeat them?

I am tightening up our electrical worksheets because a note such as “ground tested good” is not useful to the next technician. My current entry records the two meter points and the displayed voltage, but it often misses the operating condition that produced the reading.
For example, a starter ground path might show 0.04 V with the key on and 0.72 V during crank. I want a compact format that works for starting, charging, lighting and module power circuits without turning every repair order into a page of prose. What fields do you consider essential, and do you record meter polarity when a negative reading is meaningful?
My minimum record is: symptom or test objective; exact meter lead locations; circuit state and applied load; measured value; specification or comparison value; and the conclusion that the reading supports. I also note battery voltage during the test because a 0.4 V drop means something different at 12.5 V than it does during a weak 8.5 V crank.
For direction-sensitive tests I write the red-lead point first and keep the sign. A useful starter entry is: “Red lead battery negative post, black lead starter housing, 0.68 V while cranking, battery 10.4 V; limit 0.30 V; excessive ground-side drop.” That is short, repeatable and does not hide the load condition.
That format fits the worksheet. I added separate boxes for the load condition and battery voltage, plus a “spec/source” field so a comparison is not presented as a universal limit. The first repeat test by another technician produced the same result, which is exactly what was missing from our old notes.
