Brake inspections: inner-pad wear keeps being missed during quick checks

We have had two vehicles return with an inner pad near the backing plate even though the outer pad had been recorded as serviceable. Both quick inspections looked through the wheel opening and reported one measurement per axle. There was no pull or obvious brake noise at the original visit.
I am revising the inspection standard. What minimum observations catch inner-to-outer taper, seized slide pins and uneven rotor contact without automatically turning every routine check into a full brake teardown?
Record inner and outer pad thickness separately at each wheel, plus the thinnest observed point. A wheel-off view is usually needed when the caliper or shield hides the inner pad. Also note rotor surface condition on both faces, pad taper, boot damage and whether the caliper moves freely when service information allows that check.
A mirror or camera is useful, but the image must show the friction material rather than only the backing plate edge. If measurements differ materially across a caliper or axle, expand the inspection before quoting parts. The key is that “8 mm” never stands in for four pads that were not all seen.
We changed the form to four pad fields per axle and require a wheel-off inspection whenever the inner friction surface cannot be photographed clearly. The first week caught a sticking slide with 7 mm outside and 3 mm inside, before the customer had noise or rotor damage.
