P0087 under load: low-side pressure holds but rail pressure falls behind command

A 2015 Ford Explorer 3.5 direct-injection engine sets P0087 only during a long uphill pull. Low-side pressure holds around 58 psi, while commanded rail pressure rises near 1,800 psi and actual pressure falls to about 920 psi. The rail-pressure sensor reads close to atmospheric reference with the engine off and wiring checks are normal.
How would you distinguish a weak high-pressure pump from a control or injector-leak problem without replacing the pump from one loaded snapshot?
Graph commanded and actual rail pressure, high-pressure control command, low-side pressure and misfire data through a repeatable load. Confirm engine oil level and condition where the pump follows a cam lobe, and inspect the pump drive or follower according to service information. A leak-down or injector balance test can identify fuel escaping through an injector, while current or command behavior may show a control valve problem.
If low-side supply remains stable, injectors pass leakage checks and maximum control cannot raise rail pressure, inspect pump mechanical delivery before replacement. Follow fuel-pressure relief procedures before opening either circuit.
Injector leakage and balance were even. The pump control reached its limit while actual rail pressure continued to fall, and inspection found wear at the pump drive interface. After the specified pump and follower repair, actual pressure tracked within 70 psi of command on the same hill.
